All clients of Sullivan Counseling Services need to be set up in our scheduling system. This is handled through our office (614-696-8400). First-time clients need to complete new client forms before coming in for their first appointment in Westerville. 

You will be emailed a link to our client portal where you will be prompted to create a unique password. Your email on file and the password you create will give you access to our client portal where you can find new client forms and our online calendar in order to schedule, cancel, and reschedule appointments. Please read through and sign the online documentation and feel free to contact us if you have any questions about the fees and policies disclosed.

Please retain the log-in information you created so you can continue to access the portal in the future (below).  If you have problems signing in or using the client portal, or you forget your log-in information, please call us at 614-696-8400 for assistance. 

Cancellations:  We require 24 hours notice of cancellation prior to any appointment. Failure to provide adequate notice, except in the case of sickness or emergency, will result in a $50 fee charged for the session. 

Repeat Cancellations: At the discretion of their counselor, a client who regularly cancels appointments (even with 24 hours notice provided to the office) will be removed from the schedule and will not be able to schedule appointments in advance. Instead, they will need to call in the day of a desired appointment and see if there is any availability on the counselor's schedule for that day.

No Call/No Show Missed Appointments: Failure to show up for an appointment without calling our office in advance will result in immediate cancellation of all appointments scheduled with the counselor.  The client will then need to call in the day of a desired appointment and see if there is any availability on the counselor's schedule for that day.
