Cost of Services

Sessions are approximately 55 minutes in length and are charged as follows:

• $175 - Initial Client Consultation
• $145 - Existing Client/Standard Session

Out-of-pocket costs: You are responsible for all insurance deductibles and co-pays. In the absence of insurance, you will be responsible for the full cost of each session. All out-of-pocket amounts are due at the time of service.

Cancellations and missed appointments:  We require 24-hours notice of cancellation prior to any appointment. Failure to provide adequate notice, except in the case of sickness or emergency, will result in a full charge for the session. 

Look for the name Edward M. Sullivan or Edward Sullivan when searching your insurance provider list. We can help assess your coverage, but you are responsible to understand any deductible, co-pay, and/or out-of-network costs you may incur by contacting your insurance company prior to service. 

Currently, Sullivan Counseling Services is an in-network provider for the insurance companies listed below. If your insurance company is not listed, we may also be covered as an out-of-network provider, but your deductible and co-pay will be higher.

• Aetna
• Anthem Blue Cross/Blue Shield
• Medical Mutual
• Medicare
• Ohio Medicaid (including Aetna Better Health, Buckeye Health, CareSource, Humana Healthy Horizon, and UHC Community Plan)
• United Health Care

Please contact us if your insurance company is not listed to see if they have recently been added as we are continually working to expand this list.

Be sure to let us know if your insurance changes at any point. It's important to determine in advance if your coverage/co-pay has changed as well.
